These exercises have been shown to reduce excessive thoracic kyphosis, enhance postural endurance, and restore balance between anterior and posterior muscle flexibility. Studies involving both adolescent and older adult populations demonstrate that consistent extension-focused training improves shoulder and neck function while supporting overall spinal health.
Optimal thoracic alignment is closely linked to efficient head and neck posture. Maintaining this alignment facilitates stable eye movement, enhances balance, and reduces the risk of cervical strain. Conversely, forward head posture and increased thoracic curvature have been associated with impaired eye control, compromised balance, and greater susceptibility to neck pain. Improving thoracic extension contributes to more efficient postural control and head positioning.
Progression from Flexion to Extension
Following a focus on spinal flexion — connecting the ribs to the pelvis and expanding breath into the posterior thorax — extension forms the natural progression. Whereas flexion develops deep abdominal control and awareness, extension introduces openness, lift, and anterior strength while reinforcing posterior stability. The combination of both patterns supports balanced spinal function and overall movement efficiency.
Control and Postural Integration
Effective extension requires refined neuromuscular control. Maintaining a neutral pelvis and stable lumbar spine while lengthening through the head, neck, and shoulders demands precision and awareness. Through this practice, clients develop balanced spinal mechanics, counteract the effects of prolonged sitting and flexed posture, and restore extension capacity through the anterior chain.
From Swan Preparation to Full Back Bend
Swan Preparation establishes the foundation for safe and effective spinal extension. It focuses on stabilising the lumbar spine while mobilising the thoracic region — an area commonly restricted by daily posture patterns. As thoracic mobility improves, lumbar compression is reduced, resulting in smoother, more evenly distributed spinal extension. The full back bend integrates mobility through the cervical, thoracic, and lumbar regions, requiring coordinated flexibility and strength throughout the front of the body.
Shoulder and Ribcage Integration
In exercises such as Breaststroke and other overhead arm movements, optimal shoulder mechanics depend on smooth upward rotation and gliding of the scapulae across the ribcage. This scapulothoracic rhythm supports thoracic lift and facilitates extension. Concurrently, the deep stabilisers — including the abdominals, pelvic floor, gluteals, hamstrings, adductors, quadratus lumborum, and latissimus dorsi — provide the structural support necessary to maintain spinal length without compression.
Common Compensations and Clinical Corrections
- Posterior Pelvic Tilt or “Pubic Bone Press”
Issue: Excessive posterior pelvic tilt limits hip extension and encourages gluteal overactivity.
Correction: Encourage length through the front of the hips rather than pressing the pelvis into the mat. Using a Franklin ball under the hips or lower abdomen may assist in identifying pelvic neutrality and facilitating hip extension. - Rib Cage Collapse
Issue: Excessive pressure through the lower ribs restricts thoracic lift and disrupts respiratory mechanics.
Correction: Utilise lateral and posterior rib breathing to expand the ribcage evenly. Cue the sternum to reach forward while maintaining abdominal support. - Lumbar Dominance
Issue: When upper spinal mobility or anterior chain flexibility is limited, extension is often driven primarily from the lumbar spine.
Correction: Focus on initiating movement from the mid-thoracic region. Incorporate anterior chain stretches, such as Single and Double Leg Kick, to balance flexibility and reduce lumbar loading. - Cervical Overextension
Issue: Leading with the head creates cervical strain and inhibits true thoracic motion.
Correction: Guide movement from the sternum, maintaining a long cervical spine and neutral gaze. - Shoulder Elevation or Collapse
Issue: Limited shoulder mobility or stability can result in shoulder hiking or collapse during overhead patterns.
Correction: Emphasise scapular upward rotation and control within a comfortable range of motion. Progressively strengthen the shoulder complex through modified Swan or Breaststroke Preparations before advancing to full extension.

Spinal extension is not simply “back bending.” It represents the integration of length, lift, and stability across the entire kinetic chain.
